The Umass form is required if you as an applicant have ever lived in on-campus housing. This allows us to verify with the Dean of Students office your current status and inform us of any violations.

The Rental application is required of all people 18 years and older that will be considered living in the rental full time. This is information will include Applicant information, Landlord contact information, as well as credit references. Please fill in all the information. If a section does not pertain to you please draw a line through the blank area or write "N/A" so we know there is no information to check.

The Cosigner will be required if as a an applicant does not meet the income qualification in their own. The Cosigner form is completed by the cosigner themselves. All cosigners must be a Massachusetts resident and meet the income guidelines that the applicant did not. This form must be notarized and retuned to our office as part of the application processed. The cosigner will not take the place of a poor reference; its function is to prove income.
